皮质下白质创伤性轴索损伤后损伤皮质脊髓束的恢复

摘要:

皮质脊髓束是人类大脑中重要的运动通路,其对于维持手部运动功能十分关键。弥散张量纤维束成像(DTT)可使皮质脊髓束的状态可视化。但目前尚未见皮质下白质创伤性轴索损伤后损伤皮质脊髓束的情况的报道。此次,我们报道了1例54岁男性脑创伤患者,该患者右侧额颞叶出血,失去意识30min,创伤后失忆2周。四肢瘫且左手指伸肌肌无力较右手严重。伤后2周-11个月,患者进行了约9.5个月的康复治疗。伤后2周和11个月进行2次DTT检查。伤后2周DTT检查显示,皮质下白质右侧皮质脊髓束较左侧皮质脊髓束损伤严重:右侧皮质脊髓束几乎完全不连续,左侧皮质脊髓束只是部分不连续。我们认为这种表现的皮质脊髓束损伤是由于创伤性轴索损伤所致。伤后11个月,四肢肌力几乎恢复至正常,且DTT检查显示,右侧大部分不连续的皮质脊髓束恢复连续性。DTT检查结果与肌力损害和恢复情况相符。虽然已有研究应用DTI证实创伤性轴索损伤的恢复,但此次报道首次应用DTT证实了皮质下白质创伤性轴索损伤致皮质脊髓束恢复。
