Abstract: Reducing the damage to retinal ganglion cells (RGCs) or replacing them has become the goal of optic nerve protection therapy in glaucoma, which remains the leading cause of irreversible blindness worldwide. The extracellular vesicles (EVs) originated from the mesenchymal stem cells (MSCs) enhanced the survival of RGC and promoted axon regrowth in animal glaucoma models. This study studied the neuroprotective effect of MSC-EVs from the rat umbilical cord in the rat eyes with glaucoma. Chronic ocular hypertension (COH) was established on the rat eyes by injection of conjunctival fibroblasts to the anterior chamber to mimic the neural damage in glaucoma. One week after COH induction, the EVs from cultured umbilical cord-derived-MSCs were collected and injected into the vitreous cavity, and they were found to significantly attenuate the retinal damage, inhibit the RGC apoptosis associated with the down-regulation of caspase-3 expression, and reverse the RGC loss. Our results suggest that treatment with EVs derived from rat UC-MSCs promotes the survival of RGCs in rat eyes with COH/glaucoma.
