摘要:

弥漫性轴索损伤是在外力作用下颅脑加速度运动过程中造成的轴索和小血管损伤。弥漫性轴索损伤是一种严重的闭合性脑损伤,是引起死亡、严重残疾及的主要原因,尤其合并明显出血的患者与无明显出血的患者相比较其预后更差。因此明确有无弥漫性轴索损伤出血灶及其具体位置对预测病情有重要价值。如何才能及时准确的诊断弥漫性轴索损伤出血灶呢?尽管传统CT和常规磁共振成像早已应用于弥漫性轴索损伤的诊断,但他们对较小的出血灶检查并不敏感,不能满足弥漫性轴索损伤早期诊断的要求。随着磁共振成像技术的发展,特别是磁敏感加权成像序列的应用使得弥漫性轴索损伤小出血灶的检出率较以往有了很大提高。
文章回顾性分析25例经临床和影像证实的出血性弥漫性轴索损伤患者的磁共振影像资料。旨在探讨磁敏感加权成像是否对出血性弥漫性轴索损伤的评价具有较高的敏感性?是否可以更加精确地评估病情的严重性?是否可成为弥漫性轴索损伤的临床诊断治疗提供快捷、可靠影像学依据的金标准?
