摘要:

疼痛常与焦虑和抑郁等情绪障碍共病。前扣带皮质中过度兴奋性与抑制性γ-氨基丁酸能神经传递受损可引起的疼痛和疼痛相关的负面情绪。此次综述总结前扣带皮质的主要环路(包括前扣带皮质的输入和输出连接核团)、γ-氨基丁酸能神经元的分类及其功能、影响γ-氨基丁酸能神经元的神经递质和神经调制、γ-氨基丁酸能神经元与其他神经元的交接等在疼痛-情绪共病中的作用。了解γ-氨基丁酸能神经元在疼痛相关精神合并症中的作用可能有助于对疼痛和情绪合并症更有效治疗的探索。然而,调节γ-氨基丁酸能系统的机制,无论是在突触前还是突触后水平,到目前为止仍然难以捉摸。进一步探索γ-氨基丁酸能系统的复杂性可为研究和药物开发开辟新的方向。

Abstract: Pain is often comorbid with emotional disorders such as anxiety and depression. Hyperexcitability of the anterior cingulate cortex has been implicated in pain and painrelated negative emotions that arise from impairments in inhibitory gamma-aminobutyric acid neurotransmission. This review primarily aims to outline the main circuitry (including the input and output connectivity) of the anterior cingulate cortex and classification and functions of different gamma-aminobutyric acidergic neurons; it also describes the neurotransmitters/neuromodulators affecting these neurons, their intercommunication with other neurons, and their importance in mental comorbidities associated with chronic pain disorders. Improving understanding on their role in pain-related mental comorbidities may facilitate the development of more effective treatments for these conditions. However, the mechanisms that regulate gamma-aminobutyric acidergic systems remain elusive. It is also unclear as to whether the mechanisms are presynaptic or postsynaptic. Further exploration of the complexities of this system may reveal new pathways for research and drug development.
